Things to Remember for 2010 - 2011

  1. IF YOU DO NOT MAKE ANY CHANGES, YOU WILL HAVE THE SAME SELECTIONS AND WAIVERS THAT YOU DID FOR LAST SCHOOL YEAR. You can make changes August 2nd-27th only.


  2. All qualifying emplloyees remember to PRINT OFF YOUR TRS ACTIVE CARE APPLICATION OR WAIVER and physically return it to Jennifer Moody in the business office. Failue to do so may interfere with your ability to havee health insurance through TRS and/or make changes. With our online enrollment system, you can fill in your TRS form, but YOU MUST PRINT IT OUT AND TURN IT IN.

  3. Employees can view the online system to see their current benefit selections. If any changes need to be made, please fill out the TRS application and return to Jennifer Moody.

    WITH OUR ONLINE ENROLLMENT SYSTEM, YOU CAN FILL IN YOUR TRS FORM, BUT YOU MUST PRINT IT OUT AND TURN IT IN.

  4. Please call Bryan C. Keathley of Safe Harbor Benefits at 817-226-3372 for other supplemental Cancer, Heart/Stroke or Accident products or items such as:
    403Bs
    Individual Blue Cross Health Insurance
    Other Supplemental Cancer or Hearth/Stroke Policies not listed

  5. United Concordia is the new Dental provider for Caddo Mills ISD. Plan summary with schedules and rates are available under Dental in the Benefits & Forms section. After shopping the dental rates this year we have found a cost savings option with United Concordia for 2010-2011. Any person currently enrolled, will NOT have waiting periods for major services with the new plan starting 2010-2011

  6. Ameritas/ is the Vision provider for Caddo Mills ISD. Plan summary is available under Vision in the Benefits & Forms section. The Rates are not changing for 2010-2011.

  7. The Unum Educator Select Disability plan is the Disability plan option this year for Caddo Mills ISD. Employees can enroll in the plan on a guarantee issue basis. Plan brochure and rates are available under Disability in the Benefits & Forms section. The Rates are not changing for 2010-201.

  8. APL is the Cancer provider for Caddo Mills ISD. Plan summary is available, click on Benefit Highlights / Cancer. The rates are not changing for 2010-2011.

  9. APL is the MedLink provider for Caddo Mills ISD. Plan summary is available, click on Benefit Highlights / MedLink.

    The rates are not changing for 2010-2011.

  10. APL is the Heart/Stroke provider for Caddo Mills ISD. Plan summary is available, click on Benefit Highlights / Heart/Stroke.

    The rates are not changing for 2010-2011.

  11. LoneStar 529 is the College Savings plan being offered again this year. This money can be used for in or out of state school and public or private schools. The minimum amount to start a plan is $50 per month.
